PARTICIPANT WAIVER, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK

PLEASE READ THIS DOCUMENT CAREFULLY BEFORE REGISTERING YOUR CHILD. BY COMPLETING REGISTRATION, YOU AGREE TO ALL TERMS BELOW.
I, the undersigned parent or legal guardian, hereby acknowledge, agree, and consent to the following on behalf of my minor child ("Participant"):
1. ASSUMPTION OF RISK
I understand and acknowledge that participation in flag football, athletic conditioning, and related training activities involves inherent risks of physical injury including but not limited to sprains, strains, muscle tears, fractures, concussions, heat-related illness, and other injuries, some of which may be serious or permanent.
I voluntarily assume all risks associated with my child's participation in any and all Shock Elite Academy Corp programs, activities, practices, games, clinics, and events.
2. RELEASE OF LIABILITY
In consideration of my child's participation in Shock Elite Academy Corp programs, I hereby release, waive, discharge, and covenant not to sue Shock Elite Academy Corp, its officers, directors, coaches, volunteers, employees, agents, and representatives (collectively "Released Parties") from any and all claims, demands, damages, losses, or causes of action arising out of or related to my child's participation in any Shock Elite Academy Corp program or activity, including claims arising from the negligence of the Released Parties.
This release is intended to be as broad and inclusive as permitted under the laws of the State of Florida.
3. MEDICAL AUTHORIZATION
I authorize Shock Elite Academy Corp staff to seek emergency medical treatment for my child in the event I cannot be reached. I understand that I am responsible for all medical costs incurred.
I confirm that my child is physically capable of participating in athletic activities and has no medical conditions that would prevent safe participation, or that I have disclosed all relevant medical conditions to Shock Elite Academy Corp in writing prior to participation.
4. PHOTO AND MEDIA RELEASE
I grant Shock Elite Academy Corp permission to photograph, record, and use images and video of my child for promotional, educational, and fundraising purposes including but not limited to the Shock Elite Academy Corp website, social media platforms, and marketing materials, without compensation.
5. CODE OF CONDUCT AGREEMENT
I agree to ensure my child adheres to the Shock Elite Academy Corp code of conduct. I agree to conduct myself as a parent or guardian in a respectful manner toward coaches, officials, athletes, and other families at all times. I understand that violation of the code of conduct may result in my child's removal from the program without refund.
6. FLORIDA LAW
This waiver is governed by the laws of the State of Florida. If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.
7. ACKNOWLEDGMENT
By completing registration for any Shock Elite Academy Corp program, I acknowledge that I have read this Waiver, Release of Liability, and Assumption of Risk, that I understand its terms, and that I agree to be bound by it on behalf of myself and my minor child.
